The Concern

Most people assume cognitive decline is something that happens after 70. The research says it starts in your 40s — and the decisions you make in the next few years determine the trajectory.

Here are the four mechanisms driving it, and the protocol the evidence supports.

The Science

Your NAD+ is half what it was at 20

NAD+ is a coenzyme your cells depend on for energy production, DNA repair, and neuron survival. A 2026 study in Frontiers in Aging Neuroscience confirmed that brain NAD+ decline directly contributes to neurological aging — and that tissue level decline in the brain matters more to long term cognitive health than blood levels alone.

The human trial data is now substantial. A 2022 randomized double blind placebo controlled trial of 80 middle aged adults found that NMN supplementation for 60 days raised NAD+ significantly, improved physical performance, and prevented measurable biological aging. The placebo group aged detectably over 60 days. The NMN groups did not.

Your brain is running low on its most critical mineral

A 2025 randomized double blind placebo controlled trial found that 6 weeks of Magnesium L Threonate supplementation reduced estimated brain cognitive age by 7.5 years — with improvements in working memory, episodic memory, and reaction time. Most magnesium supplements do not cross the blood brain barrier. Magnesium L Threonate, specifically the Magtein form developed at MIT, is the exception.

Most adults consume well under half the magnesium their brain needs daily.

Your neurons are structurally dependent on DHA

Docosahexaenoic acid makes up approximately 97% of the omega 3 fat in neuronal membranes. It regulates synaptic plasticity, reduces neuroinflammation, and maintains the membrane structure that allows neurons to communicate. A 2024 meta analysis of 26,881 participants across 14 randomized controlled trials confirmed cognitive benefits with sustained, adequate DHA intake. The research consistently favors DHA dominant formulations over standard EPA dominant fish oil for brain specific outcomes.

Sulforaphane activates your brain's own antioxidant system

Sulforaphane, derived from broccoli sprout concentrate, activates NRF2 — the master regulator of cellular antioxidant defense. A 2025 review in Frontiers in Cellular Neuroscience identified NRF2 activation as the central mechanism behind sulforaphane's neuroprotective effects, with human studies showing improvements in mood and cognitive function in healthy older adults. The NRF2 and NAD+ pathways are complementary — both protect mitochondria and reduce neuroinflammation through different mechanisms.

The Protocol

Address all four pathways simultaneously using products that match the dosages studied in the clinical trials.

For the NAD+ precursor, the 2022 RCT used 600mg NMN daily. The 2025 NR Alzheimer's biomarker trial used 1g NR daily. A combined NMN formula that also includes resveratrol and TMG covers multiple longevity pathways in one product.

For DHA, look for a DHA dominant formulation at minimum 600mg DHA per serving. Standard EPA dominant fish oil does not replicate the brain specific benefits.

For Magnesium L Threonate, the 2025 trial used 2,000mg Magtein daily. Standard magnesium forms do not cross the blood brain barrier the same way.

For sulforaphane, the human studies used standardized broccoli sprout concentrate at 800mcg sulforaphane.
